Injury and Insurance
If you find yourself unfortunately injured during the course of an approved football activity at Mt Colah Football, you are able to log an injury claim through the NSW Football Insurance scheme.
Please follow these instructions:
1. Please notify us by sending an email to and including these details:
- Players Name
- Date of Injury
- Time of Injury
- Ground where injury occurred
- Description of injury
- Type of first aid received
- Was an ambulance called or did the player seek medical treatment after ie. Emergency ward
2. Log into the claim portal with the details you registered with here.
3. Download the club declaration form and send to and so the club can sign and send back to you
4. Upload all documents as required by the insurance and complete your claim
